Therapist/Counselor

The Therapist/Counselor with RISE Youth & Family Services develops meaningful, therapeutic relationships with individuals, families, and organizations to address identified mental health and psychoeducational needs. Grounded in a mission of fostering resilience, independence, safety, and education, this role provides counseling, education, and support services in both traditional and non-traditional settings, including homes, community spaces, and partner agencies.

Key Responsibilities

Clinical Services

  • Provide counseling and therapeutic support to assigned individuals and families
  • Complete comprehensive psychosocial assessments
  • Develop, implement, and monitor individualized treatment plans
  • Deliver psychoeducation to support client growth and understanding

Care Coordination & Collaboration

  • Coordinate care with multidisciplinary team members and community partners
  • Communicate effectively with clients, families, and other professionals
  • Attend client, school, and agency meetings as needed

Documentation & Compliance

  • Complete all required documentation, including assessments, treatment plans, and progress notes, in a timely manner
  • Maintain compliance with agency, state, and regulatory standards
  • Obtain and maintain required credentials (e.g., MA and NPI numbers)

Professional Development & Engagement

  • Complete 20 hours of initial training and 10 hours of ongoing annual training
  • Attend required staff meetings and clinical supervision
  • Stay current on best practices in mental health and trauma-informed care

Education Requirements

  • Master’s Degree in Social Work, Counseling, or a related field required
  • Current Wisconsin state licensure (in-training or fully licensed) as appropriate to degree and training

General Requirements

  • Eligible to obtain MA and NPI numbers
  • Must successfully complete all required background checks
  • Valid driver’s license with a good driving record
  • Must have a safe and reliable vehicle and proof of auto insurance
  • Ability to work in home, community, and agency-based settings
  • Strong organizational, communication, and time management skills

Preferred Qualifications

  • Experience working with youth and/or families
  • Experience in community-based or non-traditional therapy settings
  • Familiarity with trauma-informed and strengths-based approaches

Crisis and Resilience Coach (Part-Time)

The Crisis and Resilience Coach with RISE Youth & Family Services and/or Project-Mosaic builds supportive relationships with youth, families, and community partners to address identified needs. Grounded in a mission of fostering resilience, independence, safety, and education, this role provides crisis intervention, coaching, mentoring, and psychoeducational support. Services are delivered in home, school, and community settings, with a focus on immediate stabilization and long-term skill development.

Schedule

  • Minimum of 20 hours per week
  • Flexible schedule, including evenings and/or weekends as needed
  • On-call rotation crisis response required

Key Responsibilities

Crisis Response & De-escalation

  • Provide crisis intervention and de-escalation support in home, school, and community settings
  • Respond to on-call crisis situations in a timely and professional manner
  • Assess safety and support immediate stabilization

Coaching, Skill Building & Mentoring

  • Support youth and families in building coping, emotional regulation, and problem-solving skills
  • Provide mentoring to promote independence and resilience
  • Assist in developing and working toward individualized goals

Group Facilitation

  • Plan and facilitate psychoeducational and skill-based groups
  • Create a safe, engaging environment that supports learning and growth

Coordination & Collaboration

  • Coordinate with multidisciplinary team members, families, and community partners
  • Attend client, school, and agency meetings as needed
  • Participate in care planning and progress reviews

Documentation & Compliance

  • Complete all required documentation in a timely and accurate manner
  • Maintain compliance with agency and regulatory standards

Education Requirements

  • High School or GED
  • Bachelor’s Degree in Social Work, Psychology, Human Services, Education, or related field preferred
  • Equivalent experience may be considered based on qualifications

General Requirements

  • Group facilitation experience preferred
